Ethical and Policy Factors in Care Coordination (POWER POINT WITH SPEAKER NOTES)

INSTRUCTION- Select a community organization or group that you feel would be interested in learning about ethical and policy issues that affect the coordination of care. Then, develop and record a 10-12-slide, 20-minute presentation, with audio, intended for that audience. Create a detailed narrative script or speakers notes for your presentation, 4-5 pages in length.

INTRODUCTION– This assessment provides an opportunity for you to develop a presentation for a local community organization of your choice, which provides an overview of ethical standards and relevant policy issues that affect the coordination of care. Completing this assessment will strengthen your understanding of ethical issues and policies related to the coordination and continuum of care, and will empower you to be a stronger advocate and nursing professional.

PREPARATION– It would be an excellent choice to complete the Vila Health: Ethical Decision Making activity prior to developing the presentation. The activity provides a helpful update on the ethical principles that will help with success in this assessment.

Scenario

Your nurse manager at the community care center is well connected and frequently speaks to a variety of community organizations and groups. She has noticed the good work you are doing in your new care coordination role and respects your speaking and presentation skills. Consequently, she thought that an opportunity to speak publicly about contemporary issues in care coordination would be beneficial for your career and has suggested reaching out to a community organization or support group to gauge their interest in hearing from you, as a care center representative, on a topic of interest to both you and your prospective audience.

You have agreed that this is a good idea and have decided to research a community organization or support group that might be interested in learning about ethical and policy issues related to the coordination of care. Your manager has suggested the following community organizations and support groups, but acknowledges that the choice is yours.

· Homeless shelters.

· Local religious groups.

· Nursing homes.

· Local community organizations (Rotary Club or Kiwanis Club).

To prepare for this assessment, you may wish to:

· Research your selected community organization or support group.

· Review the 
Code of Ethics for Nurses With Interpretive Statements and associated health policy issues, specifically, the ACA.

· Review the assessment instructions and scoring guide to ensure you understand the work you will be asked to complete.

· Allocate sufficient time to rehearse your presentation before recording the final version for submission.

Recording Equipment Setup and Testing

Check that your audio speaker and PowerPoint software are working properly. You can record audio directly to your slides, using PowerPoint or other presentation software.

Instructions

For this assessment:

· Choose the community organization or support group that you plan to address.

· Develop a PowerPoint with typed speaker notes (the script for your voice recording) and audio voice-over recording, intended for that audience. Video is not required.

Note: PowerPoint has a feature to type the speaker notes directly into the presentation. You are encouraged to use that feature or you may choose to submit a separate document. See

For this assessment, develop your presentation slides and speaker notes, then record your presentation. You are 
not required to deliver your presentation to an actual audience.

Presentation Format and Length

You may use PowerPoint (recommended) or other suitable presentation software to create your slides and add your voice over. If you elect to use an application other than PowerPoint, check with your faculty to avoid potential file compatibility issues.

Be sure that your slide deck includes the following slides:

· Title slide.

· Presentation title.

· Your name.

· Date.

· Course number and title.

· References (at the end of your presentation).

Your slide deck should consist of 10–12 slides, 
not including a title and references slide with typed speaker notes and audio voice over. Your presentation should not exceed 20 minutes.

Create a detailed narrative script for your presentation, approximately 4–5 pages in length.

Supporting Evidence

Cite 3–5 credible sources from peer-reviewed journals or professional industry publications to support your presentation. Include your source citations on a references page appended to your narrative script.

Leadership and a Healthy Work Environment

    Leadership involves the relationship between a leader and followers with the behaviors or attributes of the leader influencing whether employees consider a work environment to be healthy. Various theories exist about the way leaders and followers construct a social order in organizations leading to a healthy work environment (Vidman & Stromberg, 2020). My perception of a healthy work environment is one in which employees are satisfied with their work, work towards a common goal, and there is no conflict with managers or other employees. Leadership is the single most important factor influencing the employee’s work environment in an organization.

    One insight from the literature about leadership is that managers who inspire workers to do their best contribute to a healthy work environment. Transformational leadership theory proposes that the way leaders use a positive relationship with followers can inspire others to adopt a common goal and voluntarily work towards achieving the goal (Pourbarkhordari et al., 2016). An element of transformational leadership is inspirational motivation by the leader that involves encouraging and motivating employees. The concept of transformational literature gave me insight into the way a manager at a part time job where I worked was able to motivate employees. The manager was always optimistic about the future even when the job we were assigned seemed impossible to do in the time allowed. The manager helped us understand that if the staff worked together as an efficient team, we could meet the deadline even though it seemed difficult. The manager also showed us how to perform tasks more rapidly. Because of these behaviors, we felt like a team and were always focused on helping each other. Even though the work was difficult, the work environment seemed positive and friendly.

    I gained another insight about leadership from literature focusing on authentic leadership theory. The theory defines authentic leadership as altruistic and transparent behaviors, which also has an association with a healthy work environment (Giordano-Mulligan & Eckhardt, 2019). Authentic leaders consistently act on their values and beliefs and demonstrate integrity. The leadership approach also involves followers in decision-making about the best approach to perform tasks. On another part-time job that I once had, the manager was responsible for a group of employees that work her floor. A rule set by the manager was that all employees should be on time for work to avoid unfairly burdening the other employees. One of the employees, however, was always late. The manager terminated the employee and took the employees place until a replacement could be hired. The manager created a healthy work environment by fairly addressing a personnel problem consistent with stated values and beliefs while minimizing the burden of the decision on other employees.

      News media has a significant influence on American citizens' lives. People rely a lot on the information received from the media, even though it may be partially accurate. In contrast with traditional news media, when things were much more straightforward, transparent, and honest, nowadays, there is so much unclear information out there that creates a vague and contradictory image of the government and politics. Since the traditional news media started to be owned by different corporations and organizations, the news has been shaped to favor a particular political party. It is acceptable that citizens have different political opinions because we are not the same, but when there is misleading and misinformation from news media, that is a serious problem. It does not help our society; contradictory, it promotes confusion and hostility among citizens.

    Most people need more time or skills to research facts, and they place significant trust and confidence in the news media to relay accurate information. I agree that citizens are uninformed about the government's standing and wait until a major issue shakes our society to voice their concerns or opinions. In 2018, Greenberg and Page stated,” individual Americans do not care a great deal about politics and are rather poorly informed, unstable in their views, and not much interested in the political process “(p. 10). Citizens are tired and frustrated with political uniformity, discouraging them from participating in the political process. But how can we change this? It is very complex. More restrictions and legal sanctions must be imposed on news channels and stations that report fake news. In addition, citizens must be accurately informed about the facts affecting our society and the political leaders' involvement in governance so they can make well-informed voting decisions.

     Here, in the state of Florida, one of the main barriers was the clause that nurse practitioners must work on the sponsorship of a physician or within close proximity of a physician. That has been relaxed by legislation that says only nurse can work in most sectors of healthcare, without oversight of a physician, this allows APRNs autonomy in a medspa, primary care clinics and/or even telemedicine when working independently and as an entrepreneur.

    Since June of 2020, Florida operates under “Full practice”policy, which allows nurses that have proof of at least 3000 clinical hours of experience and graduate level pharmacology and differential diagnosis courses, the privilege to operate autonomously in primary care, med spas and even telemedicine under this regulation. This is considered to be the gold standard as APRNs are now able to register as an Autonomous Advanced Practice Registered Nurse and practice to the full extent of their role (Fla. BON, 2023) 

     With just just three short years ago, APRNs had to work under the supervision or sponsorship of a physician that was either either working in the practice or had a vested interest in a clinic where the APRN is practicing. Some states may also hold a prescriptive regulation or the APRN whereas they can only order certain schedules and medications. In some instances there may be pushback from physicians who do not co-sign the concept of nurse practitioners and or having the privileges that they are allowed believing that they are not trained like physicians. In some communities nurse practitioners may not be a choice of patients to be examined by and may just prefer their doctor instead.

     For midwives in Florida, some of the barriers include the need to have a supervisory agreement with a Florida licensed physician for post graduate supervisory hours A lack of recognition as primary care providers on Medicaid and Medicare and private insurance company provider panels and other restrictions. (Hastings, et al, 2018)

     CRNAs have barriers such a lack of fair reimbursement by third-party, payers,  stating that they are not trained on the same skill level as physicians and failure of some of the other surgical groups or colleagues to recognize CRNAs as board certified to administer anesthesia. (Toney, 2023)

     Rep. Cary Pigman (R-Sebring) and Sen. Jeff Brandes (R-St. Petersburg) joined members of the interest groups for APRNs the Florida Association of Nurse Anesthetists (https://www.fana.org/)and the Florida Nurse Practitioner Network (https://fnpn.enpnetwork.com/) in making new legislation that forwards, the causes nurse practitioners and pushes for more independence for them as well.

In addition to the dissemination of information and evidence-based findings and the development of tool kits, continuous support for and availability of such resources are critical. Among the most popular methods to promote ongoing dialogue and information sharing are blogs, wikis, websites, and social media. Nurses know how to support people in time of need or crisis and how to support one another in the workplace; wikis in particular enable nurses to continue that support beyond the work environment. Here they can be free to share their unique perspectives, educate others, and promote health care wellness at local and global levels (Kaminski, 2016).You are encouraged to complete the Determining the Relevance and Usefulness of Resources activity prior to developing the repository. This activity will help you determine which resources or research will be most relevant to address a particular need. This may be useful as you consider how to explain the purpose and relevance of the resources you are assembling for your tool kit. These resources should enable nurses and others to implement and maintain the safety improvement you have developed.It is recommended that you focus on the 3 or 4 most critical categories or themes with respect to your safety improvement initiative.
