For this week utilize the tool listed above to complete the assignment.

Overview

As you will learn throughout the program, the diagnosis of a variety of psychiatric illnesses is not always an easy or straightforward process. Multiple observations and assessment methods are often employed to reach a diagnosis. This approach can include the use of standardized assessment instruments.  This then aids you in defining a treatment plan and choosing specific treatment plans to use in the care of your clients.

You are tasked with
identifying a standardized assessment instrument/tool to measure the disorders listed for each week. You will keep these instruments in the form of a “portfolio” that you can use in your clinical practice to assess clients who present with a variety of symptoms.

Instructions:

Instrument/ Tool criteria:

For each assessment, you are tasked with selecting, you will identify an instrument and:

1. List what
DSM diagnosis the tool/instrument is used for.

2.
Identify an assessment/diagnosis instrument.

3. Appraise a scholarly, peer-reviewed article that addresses the use of the instrument to support your choice as an evidence-based instrument for practice.

4. Evaluate the instrument’s appropriateness for diagnosing the condition it is designed to assess or if the developers of the instrument reported that the instrument is only part of a comprehensive assessment for the disorder.

5. Describe whether or not the instrument can be used to measure patient response to therapy/treatment or if it is strictly for assessment and diagnosis.

6. Discuss the psychometrics/scoring of the instrument, including reliability and validity.

7. Discuss any limitations associated with the use of the instrument.

8. Include a link to view the assessment if possible.

Use the following template in completing your portfolio assignments. Your information can be in a bulleted format or just a couple of sentences for each criterion listed above. However, you must use APA citations.  You are NOT required to write this in a paper format.  Turn in one document for each week’s topics.  (However, create a file on your desktop to compile your portfolio as you move through the term.)  This will ensure you can have easy access to show the full portfolio and once you begin clinicals and practice.  Throughout the program, you will continue to add to the portfolio in each course.

Anxiety and Related Disorders

Week 4

DSM: Generalized Anxiety Disorder

Instrument: Generalized Anxiety Disorder 7 (GAD-7) and Generalized Anxiety Disorder 2

(GAD-2)

Article: Validation and Standardization of the Generalized Anxiety Disorder Screener (GAD-7)

in the General Population. Medical Care

Appropriateness for Dx: The GAD-7 and GAD-2 is screening tools for initial assessment for

mental health screening. The screening is the process used by healthcare provider to assess the

need to further evaluate for anxiety disorders. This helps determine the need for further

questioning on the anxiety the individual may be experiencing.

Response to Therapy/Treatment: This screening is effective for self-reporting of anxiety for

the healthcare provider. The evidence supports the validity and reliability of the GAD-7 and

GAD-2 for screening for anxiety disorders (Löwe et al., 2008).

Assessment Instruments

Psychometrics: The questionnaire is a self-reporting tool used for the individual to report any

problematic areas dealing with anxiety. The screening tool has 7 questions in the GAD-7 and the

shorter briefer questionnaire has 2 questions. The questionnaire asks the individual “Over the last

2 weeks, how often have you been bothered by the following problems?”. Then list 7 different

items relating to anxiety and the GAD-2 is the same just stops after question 2. The is effective

screening tool to help see the problem areas. The individual has several different choices to

provide feedback. It is on a scaled system to calculate a score. For example, the first choice “0”

has the words “not at all” and the last choice is “nearly every day” with the score of “+3”. The

calculated score at the end is broke down by minimal, mild, moderate, severe anxiety based on

the scoring.

Limitations:

Limitations: The limitations on the screening tool is the individual answering truthfully or to the

best of their ability. The assessment and further evaluation by the healthcare provider is

necessary for validation of the diagnosis.

Interdisciplinary teams

We have now learned about a number of different careers in Health Professions. What should be evident is that no one in health works alone. Interdisciplinary teams are key to providing good care for patients. An interdisciplinary team is ” a group of healthcare providers from different fields who work together or toward the same goal to provide the best care or best outcome for a patient or group of patients.”

In 2 paragraphs (3-5 sentences each) please describe an interdisciplinary healthcare team as you envision it. Be specific, list 4 different healthcare professionals you would see treating a patient in the hospital. (You can decide the illness the patient has to give you freedom to be creative with your team. ) Then describe their role on the team and how they work together.

sociology discussion 6

Respond to two (2) of the following prompts:

  1. Describe formal and informal education. From your experiences with schooling, provide specific examples of each and explain how they are built into the curriculum or culture of the school. (USLO 6.1)
  2. Reflecting on your own experience (or your child’s) with schooling, explain one sociological theory that best relates to your (or your child’s) academic journey. Describe two examples of how this theory applies to your schooling. (USLO 6.4)
  3. What do you think is the most significant concern with the education system today? Provide a suggestion for how this issue could be improved. (USLO 6.3)
  4. What is the difference between mainstreaming and inclusion? Which do you think is the optimal way to include people with disabilities in the classroom and why? (USLO 6.2)
  5. Karl Marx studied the social impact of religion and believed religion reflects the material base, which is comprised of the means and modes of production and the relations of production. Marx argued that religion maintains inequality and perpetuates the status quo. Is Marx’s argument that religion maintains inequality and perpetuates the status quo true? Present an example that supports your assessment of Marx’s argument. (USLO 6.8)
  6. Emile Durkheim argued that religion promotes social cohesion, and social control, as well as offering meaning and purpose to human life. Is Durkheim’s argument true? Present an example that supports your assessment of Durkheim’s argument. (USLO 6.8)
  7. What is required of a society for its political structure to be considered democratic? While this question might seem straightforward, it is not and is a topic of contentious debate in current political philosophy circles. The Democracy Index lists 60 different indicators that must be considered when discerning whether a nation is a democracy. While you in no way will be expected to discuss all indicators, please talk about a few of the conditions that must exist for a government to qualify as a democracy. Include textual evidence to support your view. (USLO 6.5, 6.6)
  8. In recent years in the United States, citizens have increasingly made claims that our judicial system is institutionally racist. Evidence provided for these claims has pointed toward unfair drug sentencing laws, stop and frisk policies, the use of police force, and the overwhelming number of black men in prisons and jails in comparison to the rest of the population. Select one claim and discuss whether these claims have merit, presenting specific examples to support your position. Include textual evidence to support your view. (USLO 6.7)

Dimensions of Nursing Practice

Hello Everyone,

In a health facility, it is important to ensure that patients morals and values are being obtained. Many people come from different cultural backgrounds and it is crucial to be able to understand them and respect them the best way possible. You want to make sure everyone feels safe and comfortable between the patient and the nurse. Its important to make sure the patient feels respected with whatever healthcare decisions they decide to make.

1.What values ensure a culture of safety?

Like in most situations, communication amoungs co-worker and patients is key to gather the proper information about the patient. It is crucial that we have a strong and well established team that makes working as a team a priority for every patient. We have to value everyone decisions and what they think is best for themselves. Culture plays a huge role in the patients decision making for themselves. It is important that as a team we work together to sustain a safe environment and to make sure everything is organized. Having great organization skills keeps the environment stay at a better state and avoids any problematic risk factors that can potentially harm the patient in any way.

2.How can healthcare facilities establish a culture of safety?

Some ways is establishing great teamwork and great work ethic practices. As nurses, we know to use EBP to ensure that the patient is in the best care needs from us. Working as a team will help maintain the environment the more nurses that work together the better the outcomes. Everyone in the facility should be promoting safety to make the environment welcoming.

3.What is the nurse's role in maintaining a culture of safety?

We all know that every hospital is going to have different set of rules depending on the state and region. Although, most hospitals will have very similar rules that nurses and medical professionals must follow. Many of these hospitals have to follow government advised rules like using EBP for every patient. I still think every single nurse should be educated in different cultures to understand every patient and for them to be well respected. Nurses play a huge role in taking proper care of each unique individual. Actively being part of ANA, taking every protocol seriously and maintaining a safe work environment for other staff and for the patient. As nurses, we play a vital role in creating a culture of safety within our clinical work area by maintaining an open line of communication with our healthcare team members, assessing our peers' and our own professional work habits and behaviors for safety risks, utilizing evidence-based practice (EBP) interventions in our patient protocols, and participating in shared governance workgroups (Davis,2017).

In a health facility, it is essential to produce the culture of safety. It is important for workers within the healthcare facility to have respect for one another, especially for everyone to feel safe with each other. Not only that, but it's crucial for a facility's culture to support and promote patient safety.

1. What values ensure a culture of safety?

After reading, I have learned that there are various values that ensure a culture of safety. One example is the value of teamwork. The team needs to work together in order to provide each patient with a consistent safe experience. Another value is respect. As I mentioned earlier it is important for healthcare workers to respect one another but it is just as important for healthcare workers to be nothing but respectful to their patients. Another value includes organization and planning. It is important to acknowledge what can cause the culture of safety and plan accordingly.

2. How can healthcare facilities establish a culture of safety?

Some examples include teamwork by healthcare workers. It is important for leaders to be visibly committed to change. It is also essential for the full staff to openly share safety information with one another. Everyone should be on the same page of promoting patient safety.

3. What is the nurse's role in maintaining a culture of safety?

I understand that every health facility is ran differently and has different standards. I believe though, that every nurse should maintain a culture of safety. I feel that they have a crucial role because they are the main caregivers of the patients when they come to the facility. The patient is allowing the nurse to take care of them by putting a lot of trust in them. Due to this trust, nurses need to make it a goal to keep them safe at all times. The nurses are in control as far as safety because they are aware of the facility and regulations. If the nurse does a successful job at maintaining a culture of safety, it will create a safe and trusting experience between the nurse and the patient.

reflection

1. Comment on this statement that is often heard: “I have practiced (or taught) nursing for many years without the need to use theory, so why do I need theory in a practice discipline?”

2. Reflect on this statement: “A scholarly discipline must engage in societal concerns, in dialogues about pressing issues, and in shaping health care reform.” Is nursing a scholarly discipline? Do nurse scholars have the same attributes as other scholars in other disciplines? What would be your assessment of the level of scholarship in nursing?

3. What difference do the levels and types of theory make in advancing nursing knowledge?
